Insurance Information

We understand that dealing with insurance companies can be somewhat confusing. In an effort to help you through the process, we have provided the following information.

Insurance Billing

We can provide physical therapy services directly without a referral. However many insurance companies require a physician referral or prescription to pay for services.

Please check with your insurance company to determine your Physical Therapy benefits, deductible, co-pays, and/or if a prescription is required.

We accept all major insurance carriers. For insurance companies with which we are not contracted, you may still be able to utilize your insurance coverage if you have “out of network” benefits. We suggest that you check on your “out of network” benefits to determine potential financial responsibility before attending appointments.

During your first visit, our Pro-Active PT office staff will ask you to copy your insurance card. With the information on your card together with additional information you provide during the initial visit, we will be able to verify insurance coverage and benefits. Whenever possible we will try to collect enough information prior to your first visit to verify coverage. Once we are able to verify coverage, we will provide you with an estimate of the cost to you if any. We will bill your primary insurance company for you. Please be aware most insurances require a portion of each visit to be the patient responsibility in the form of a co-pay. We require co-pays to be made at the time of service.

Medicare

Medicare requires a referral from a physician for physical therapy, which is valid for the first 30 days of treatment. After that, a new referral is required every 30 days to continue therapy. If continued care is necessary we will make every effort to obtain a new referral for you. Your physical therapist will re-evaluate your condition and send a report to your referring physician to authorize continued care. Your physician may ask you to schedule a visit with their office before authorizing further treatment.

Medicaid

We accept Medicaid insurance, for those individuals under the age of 19. Unfortunately, outpatient physical therapy is not a covered service for individuals that are 19 years of age and older.

Workman’s Comp

We do accept workman’s comp patients. Please be aware that your WKCP insurance/administrator must approve any therapy before we can begin and/or continue therapy, even with a physician referral.

Motor Vehicle Accidents

We DO NOT bill any insurance for motor vehicle accidents. If you are wanting to be seen due to an MVA this will have to be self-pay. Upon request, we will provide you copies of the therapist’s notes and receipts to send to your insurance company to be reimbursed.

Self Pay

We do offer therapy on a self-pay basis for those who do not have insurance or do not want to use their insurance. Please do not hesitate to contact us at 501-778-4960 with any questions.
